RESERVATION FEE (TO SECURE DATES)
$30/day booked - Paid at time of reservation to hold secure your dates. This total is put towards total fees quoted, not in addition to.
Eg: Book 5 full days, your reservation fee is $150
MILEAGE
$2.00/mile/day for offices beyond 25 miles from doctors home. Calculated via Google Maps from door to door
HOLIDAY
$250 Holiday fee for coverage booked that includes the day before, day of, or day after the following holidays: Christmas Day, New Year’s Day, July 4th, Thanksgiving Day.
EMERGENCY COVERAGE
If scheduling for a start date less than 3 days ahead, $200 will be added to the total quote.
LATE CANCEL FEE
If cancellation occurs less than 48 hours from the start of the first shift being covered, the reservation fee is kept PLUS 25% of the remaining total quoted coverage. This will be calculated and made clear at time of booking and be on the contract.
